ABSTRACT

Food handlers plays a primary role in the transmission of pathogenically important protozoans and helminth parasites. This study was aimed to evaluate the prevalence of intestinal pathogenic protozoans and helminth parasites among food handlers in and around University of Malakand, Lower Dir, Pakistan. Stool samples were collected from 642 food handlers (all of male) in a cross-sectional study from January to November, 2017. Wet Mount Techniques and concentration methods by using salt and formol–ether solutions. Three hundred and eighty four cases (59.8%) were found infected with one more parasites. Most of the individuals were found infected with helminth (47.6%) as compared to intestinal protozoans (0.93%). Seventy two cases (11.2%) of the cases presented mixed infection with both intestinal protozoan and helminth parasites. The order of prevalence for intestinal helminth was Ancylostoma duodenale (n = 258, 40.1%), followed by Taeniasa ginata (n=96, 14.9%) Ascaris lumbricoides (n = 54, 8.40%) and Trichuris trichura (n=30, 4.60%). For intestinal protozoa, Entamoeba histolytica/dispar (n = 36, 5.64%) was the only protozoan detected. Mono-parasitism was higher than poly-parasitism. Family size income and education level were the factors significantly (P<0.05) associated in the parasites prevalence. Current research showed that IPIs are primarily the foodborne pathogens still an important public health problem in Pakistan. Effective control programs on parasitic diseases transfer and their associated factors are recommended.


Os manipuladores de alimentos desempenham um papel fundamental na transmissão de protozoários e helmintos parasitas patogenicamente importantes. Este estudo teve como objetivo avaliar a prevalência de protozoários patogênicos intestinais e helmintos parasitas entre manipuladores de alimentos na Universidade de Malakand, Lower Dir, Paquistão. Amostras de fezes foram coletadas de 642 manipuladores de alimentos (todos do sexo masculino) em um estudo transversal de janeiro a novembro de 2017. Técnicas de montagem úmida e métodos de concentração usando soluções de sal e formol-éter. Trezentos e oitenta e quatro casos (59,8%) foram encontrados infectados com mais um parasita. A maioria dos indivíduos foi encontrada infectada por helmintos (47,6%) em comparação com protozoários intestinais (0,93%). Setenta e dois casos (11,2%) dos casos apresentavam infecção mista com protozoários intestinais e helmintos parasitas. A ordem de prevalência de helmintos intestinais foi Ancylostoma duodenale (n = 258, 40,1%), seguido por Taeniasa ginata (n = 96, 14,9%) Ascaris lumbricoides (n = 54, 8,40%) e Trichuris trichura (n = 30, 4,60 %). Para protozoários intestinais, Entamoeba histolytica / dispar (n = 36, 5,64%) foi o único protozoário detectado. Monoparasitismo foi maior do que poliparasitismo. A renda familiar e o nível de escolaridade foram os fatores significativamente (P <0,05) associados na prevalência de parasitos. A pesquisa atual mostrou que os IPIs são principalmente os patógenos de origem alimentar, ainda um importante problema de saúde pública no Paquistão. Programas eficazes de controle da transferência de doenças parasitárias e seus fatores associados são recomendados.

ABSTRACT

Tapeworms of zoonotic importance have been described as a leading public health problem. Current research was aim to assess the prevalence of tapeworms among 5-12years school children residing in district Lower Dir, Pakistan from January 2019-December 2019. The wet mount preparation in saline/iodine/methods were used for stool examination. Data was analyzed using appropriate descriptive, static methods. Of the 400 children studied 71.7% were infected with one or more species of intestinal parasites. Single infection of cestode species was found in 69 individuals with 17.2% prevalence and multiple parasitic infections were identified in 19.7% (n=79/400) individuals. The multiple infection were comprised as 10% (n=40) double, 6.75% (n=27) triple and 3% (n=12) quadruple. A total of 9 species of helminths and one species of protozoan infection. Among the helminths Ascaris lumbricoides was the most prevalent 33.1% (n=95), Taenia saginata 22.6% (n=65), hookworm 19.8% (n=57), Hymenolepis nana 18.8% (n=54), Enterobius vermicularis and Hymenolepis diminuta 1.39% (n=4each), Trichuris trichura 1.04% (n=3), Toxocara spp 0.69% (n=2) and Schistosoma japonicum 0.34% (n=1) were reported. One protozoan species was Cryptosporidium spp 0.69% (n=2) in current study. In case of A.lumbricoides, hookworm, E.vermicularis, T.trichura, T.saginata, H.nana and H.diminuta the male children of below 8 years of age were highly infected. Other infections are reported in the same prevalence with slight difference if any. We conclude that there is a need for mass scale campaigns to create awareness regarding health and hygiene in children and the need for development of effective poverty control programs because deworming alone is not adequate to control parasitic infections.


As tênias de importância zoonótica têm sido descritas como um dos principais problemas de saúde pública. A pesquisa atual teve como objetivo avaliar a prevalência de tênias entre crianças em idade escolar de 5 a 12 anos que residem no distrito de Lower Dir, Paquistão, de janeiro de 2019 a dezembro de 2019. Os métodos de preparação para montagem úmida em solução salina/iodo foram usados para exame de fezes. Os dados foram analisados usando métodos descritivos e estáticos apropriados. Das 400 crianças estudadas, 71,7% estavam infectadas com uma ou mais espécies de parasitas intestinais. Infecção única de espécies de cestóides foi encontrada em 69 indivíduos com prevalência de 17,2% e infecções parasitárias múltiplas foram identificadas em 19,7% (n = 79/400) indivíduos. As infecções múltiplas foram compostas por 10% (n = 40) dupla, 6,75% (n = 27) tripla e 3% (n = 12) quádrupla. Um total de 9 espécies de helmintos e uma espécie de infecção por protozoários. Entre os helmintos, Ascaris lumbricoides foi o mais prevalente 33,1% (n = 95), Taenia saginata 22,6% (n = 65), ancilóstomo 19,8% (n = 57), Hymenolepis nana 18,8% (n = 54), Enterobius vermicularis e Hymenolepis diminuta 1,39% (n = 4cada), Trichuris trichura 1,04% (n = 3), Toxocara spp 0,69% (n = 2) e Schistosoma japonicum 0,34% (n = 1). Uma espécie de protozoário foi Cryptosporidium spp 0,69% (n = 2) no estudo atual. No caso de A. lumbricoides, ancilostomíase, E. vermicularis, T. trichura, T. saginata, H. nana e H. diminuta, as crianças do sexo masculino com menos de 8 anos de idade estavam altamente infectadas. Outras infecções são relatadas na mesma prevalência, com ligeira diferença, se houver. Concluímos que há uma necessidade de campanhas em massa para criar consciência sobre saúde e higiene em crianças e a necessidade de desenvolvimento de programas eficazes de controle da pobreza, porque a desparasitação por si só não é adequada para controlar infecções parasitárias.

Resumen: Introducción: La inflamación asociada con la infección por Helicobacter pylori (H. pylori) se relaciona con la pro gresión de las lesiones precancerosas gástricas. Las infecciones por helmintos podrían modular la respuesta proinflamatoria a la infección por H. pylori desde un perfil tipo LTCD4+ Th1 hacia una respuesta menos perjudicial tipo LTCD4+ Th2. Objetivo: Caracterizar la polarización de la respuesta inmune tipo LTCD4+ Th1/Th2 de pacientes coinfectados por H. pylori y helmintiasis procedentes de áreas de bajo riego para el desarrollo de cáncer gástrico. Pacientes y Método: Se analizaron 63 pacientes, 40 adultos y 23 niños infectados con H. pylori. La determinación de los perfiles séricos de las interleucinas asociadas con la polarización de la respuesta inmune tipo LTCD4+ Th1 (IL-1Β, INF-γ y TNF-α) y tipo LTCD4+ Th2 (IL-4, IL-10 e IL-13) se realizó con Análisis Multiplex (xMAP). La relación entre el estado de coinfección por helmintos en pacientes infectados con H. pylori y la polarización de la respuesta inmune mediada por LTCD4+ Th1 y LTCD4+ Th2, se estudió con un modelo de regresión logístico de efectos mixtos. Resultados: La frecuencia de helmintos fue similar en adultos (15%) y niños (17%). La polarización de la respuesta inmune fue más prevalente hacia el tipo LTCD4+ Th1. Los valores séricos de las interleucinas asociadas con la polarización de la respuesta inmune tipo LTCD4+ Th1 (IL-1 Β, INF-γ y TNF-α) y tipo LTCD4+ Th2 (IL-4, IL-10 e IL-13) fueron independientes del estado de infestación por helmintos. Conclusión: La prevalencia de infección por parasitismo intestinal fue alta y la polarización de la respuesta inmune fue predominantemente hacia un perfil tipo LTCD4 + Th1.


Abstract: Introduction: Inflammation associated with Helicobacter pylori (H. pylori) infection is linked to the development of a gastric precancerous lesion. Helminth infections could influence the pro-inflam matory response to such infection from LTCD4+ Th1 to a less harmful LTCD4+ Th2 response. Ob jective: To characterize the polarization of the LTCD4+ Th2 immune response in co-infected pa tients with H. pylori and helminths from low-risk areas for developing gastric cancer. Patients and Method: We analyzed 63 patients infected by H. pylori (40 adults and 23 children). Through the Multiplex Analysis technology (xMAP), we determined the serum profiles of the interleukins asso ciated with the polarization of the immune response of LTCD4+ Th1 (IL-1Β, INF-γ, TNF-α) as well as the LTCD4+ Th2 (IL-4, IL-10, and IL-13). The ratio between helminths co-infection status in H. pylori-infected patients and the polarization of the immune response mediated by LTCD4+ Th1 and LTCD4+ Th2 was assessed using a Mixed Effects Logistic Regression Model. Results: The frequency of helminths was similar between adults (15%) and children (17%). The polarization of the immu ne response was more prevalent in LTCD4+ Th1. Serum values of interleukins associated with the immune response polarization of LTCD4+ Th1 (IL-1Β, INF-γ, and TNF-α) and LTCD4+ Th2 (IL-4, IL-10, and IL-13) were independent of helminths infection status. Conclusion: The prevalence of in testinal parasitic infection was high and the immune response polarization was mainly LTCD4 + Th1.

Introdução: Sistemas de inteligência artificial são tecnologias promissoras de assistência em saúde e diagnóstico laboratorial, que podem ser implementados como métodos de suporte para o diagnóstico de parasitoses intestinais. Este estudo objetivou desenvolver um software de IA que auxilia no diagnóstico laboratorial de parasitoses intestinais, com alta sensibilidade e especificidade. Métodos: O software foi desenvolvido utilizando duas redes neurais, Inception e MobileNet. Primeiro imagens de ovos dos parasitas Ascaris lumbricoides, Trichiuris trichiura, Taenia sp, Hymenolepis nana, Schistosoma mansoni e larvas de Strongyloides stercoralis, foram utilizados para treinar o banco de dados. Posteriormente 2.740 imagens cedidas pelo Laboratório de Parasitologia da Universidade do Oeste de Santa Catarina foram testadas no software. Resultados: O software apresentou sensibilidade de 82,3% (95% intervalo de confiança (IC), 71,9%-89,1%) e especificidade de 95,1% (95% IC, 94,3%-97,8%) para MobileNet e sensibilidade de 72,1% (95% IC, 52,6%-115%) e especificidade de 92,1% (95% IC, 91,7%-97,7%) para Inception. Conclusão: O software apresentou resultados promissores na análise de parasitas intestinais, reforçando que, no futuro, a presença de sistemas de suporte de diagnóstico das parasitoses pode vir a se tornar mais rápido e eficiente. Introduction: Artificial intelligence systems are promising technologies for health care and laboratory diagnosis, which can be implemented as support methods for the diagnosis of intestinal parasitoses. This study aimed to develop an artificial intelligence software that assists the laboratory diagnosis of intestinal parasitoses with high sensitivity and specificity. Methods: The software was developed using two neural networks, Inception and MobileNet. First, images of eggs from the parasites Ascaris lumbricoides, Trichuris trichiura, Taenia sp., Hymenolepis nana, Schistosoma mansoni and Strongyloides stercoralis larvae were used to train the database. Then, 2,740 images provided by the Parasitology Laboratory of the Universidade do Oeste de Santa Catarina were tested in the software. Results: The software had a sensitivity of 82.3% (95% confidence interval [CI], 71.9% ­ 89.1%) and a specificity of 95.1% (95% CI, 94.3% ­ 97,8%) for MobileNet and a sensitivity of 72.1% (95% CI, 52.6% ­ 115%) and a specificity of 92.1% (95% CI, 91.7% ­ 97.7%) for Inception. Conclusion: The software showed promising results in the analysis of intestinal parasites, reinforcing that, in the future, the presence of diagnostic support systems for parasitoses may become faster and more efficient. Resumen Introduction: The diagnosis of intestinal parasitic infections depends on the parasite load, the specific gravity density of the parasite eggs, oocysts or cysts, and the density and viscosity of flotation or sedimentation medium where faeces are processed. Objective: To evaluate the concordance between zinc sulphate flotation and centrifugal sedimentation in the recovery of parasites in faecal samples of children. Materials and methods: Faecal samples of 330 children from day care centers were evaluated by zinc sulphate flotation and centrifugal sedimentation techniques. The frequencies of detection of parasites by each method were determined and the agreement between the diagnostic techniques was evaluated using the kappa index, with 95% confidence intervals. Results: The faecal flotation in zinc sulphate diagnosed significantly more cases of Trichuris trichiura infection when compared to centrifugal sedimentation (39/330; 11.8% vs. 13/330; 3.9%, p<0.001), with low diagnostic concordance between methods (kappa=0.264; 95% CI: 0.102-0.427). Moreover, all positive samples for Enterobius vermicularis eggs (n=5) and Strongyloides stercoralis larvae (n=3) were diagnosed only by zinc sulphate. No statistical differences were observed between methods for protozoa identification. Conclusions: The results showed that centrifugal flotation in zinc sulphate solution was significantly more likely to detect light helminths eggs such as those of T. trichiura and E. vermicularis in faeces than the centrifugal sedimentation process.


Abstract Introducción. El diagnóstico de infecciones parasitarias intestinales depende de la carga de parásitos, la densidad de la gravedad específica de los huevos, ooquistes o quistes de parásitos, y de la densidad y viscosidad de los reactivos de flotación o sedimentación usados para procesar las heces. Objetivo. Evaluar la concordancia entre el método de flotación de sulfato de zinc y la sedimentación por centrifugación en la recuperación de parásitos en muestras fecales de niños. Materiales y métodos. Se evaluaron las muestras fecales de 330 niños de guarderías mediante las técnicas de flotación con sulfato de zinc y de sedimentación por centrifugación. Se determinó la frecuencia de detección de parásitos con cada método y se evaluó la concordancia entre las técnicas de diagnóstico mediante el índice kappa, con intervalos de confianza del 95 %. Resultados. Mediante la flotación fecal con sulfato de zinc, se diagnosticó un número significativamente mayor de casos de infección por Trichuris trichiura que con la sedimentación por centrifugación (39/330; 11,8 % Vs. 13/330; 3,9 %) (p<0,001), con poco acuerdo entre los métodos (kappa=0,264; IC95% 0,102-0,427). Además, todas las muestras positivas para huevos de Enterobius vermicularis (n=5) y larvas de Strongyloides stercoralis (n=3) se diagnosticaron solamente por sulfato de zinc. No se observaron diferencias estadísticamente significativas entre los métodos para la identificación de protozoos. Conclusiones. La flotación centrífuga en una solución de sulfato de zinc presentó una probabilidad significativamente mayor de detectar los huevos livianos de helmintos como T. trichiura y E. vermicularis en heces, que el proceso de sedimentación por centrifugación.

ABSTRACT

A criação de ovinos no Brasil vem se desenvolvendo em larga escala nos últimos anos, entretanto, as verminoses continuam causando grandes perdas no setor de ovinocultura. Dentre as verminoses, as helmintoses são responsáveis pelos maiores prejuízos na criação de ovinos, sendo o principal parasita causador destas perdas o Haemonchuscontortus, predominante em diversas regiões do Brasil. Grande parte desta patogenicidade é proveniente de sua hematofagia e lesão na mucosa do abomaso. Esta perda de sangue implica também na perda de outros elementos figurados sanguíneos. Animais com intensa parasitose podem apresentar, além da anemia, leucopenia por linfopenia e perdas proteicas que caracterizam uma hipoproteinemia. Dessa forma, métodos de avaliação do estado sanitário dos animais foram desenvolvidos como estratégia auxiliar no controle do Haemonchus. O grau de anemia dos animais do experimento foi determinado pelo método Famacha e por métodos laboratoriais: albumina, hematócrito, hemoglobina, proteína plasmática total, contagem total de eritrócitos, volume globular, leucócitos totais, linfócitos e OPG (ovos por grama de fezes) em ovelhas. O objetivo foi correlacionar o método Famacha com os resultados dos parâmetros sanguíneos, para conhecer de forma mais rápida, o estado de anemia do animal como indicador de hemoncose. Foram utilizadas para o experimento 20 ovelhas mestiças da raça Dorper. As coletas de sangue, fezes e a avaliação da coloração da conjuntiva ocular por meio do método Famacha foram realizadas no mesmo dia. Foi observado um maior número de animais com o grau de Famacha2, os animais com grau 1 apresentaram menor número de OPG, maior número de hemácias, maior quantidade de hemoglobina, maior hematócrito quando comparados com os animais com graus 2 e 3. Conclui-se que o método Famacha é eficaz na estratégia de combate da hemoncose ovina.


Sheep farming in Brazil has been developing in a large scale in recent years. However, worms continue to cause great losses in sheep production. Among them, helminthes are responsible for major losses in sheep production, with the main parasite causing such losses being Haemonchuscontortus, prevalent in several regions in Brazil. Much of this pathogenicity comes from its hematophagy and the damage it causes to the abomasum mucosa. This blood loss also means the loss of other blood-formed elements. Animals with severe parasitic disease may present, as well as anemia, leukopenia by lymphopenia and protein loss characterizing hypoproteinemia. Thus, methods of assessing the sanitation status of animals were developed as a strategy to assist in the control of Haemonchus. The degree of anemia in experimental animals was determined by the Famacha method and by laboratory methods: albumin, hematocrit, hemoglobin, total plasma protein, total erythrocyte count, erythrocyte volume, total leukocytes, lymphocytes and EPG (eggs per gram of feces) in sheep. The purpose was to correlate the Famacha method with the blood parameter results in order to have a quicker answer about the animal's anemia state as a haemonchosis indicator. A total of 20 crossbred Dorper sheep were used in this experiment. The collection of blood and feces, and conjunctival staining by the Famacha method were performed on the same day. An increased number of animals presenting Famacha grade 2 could be observed, while animals with grade 1 had fewer EPG, higher number of erythrocytes, greater amount of hemoglobin, and better hematocrit compared to animals with grade 2 and 3. It can be concluded that the Famacha method is an effective fighting strategy against ovine haemonchosis.


La cría de ovinos en Brasil viene desarrollándose a gran escala en los últimos años. Sin embargo, los helmintos siguen causando grandes pérdidas en el sector de la producción ovina. Entre las parasitosis, los helmintos son responsables por mayores pérdidas en el ganado ovino, el principal parásito que causa estas pérdidas es el Haemonchus contortus, que prevalece en varias regiones de Brasil. Gran parte de esta patogenicidad proviene de su hematofagia y lesión de la mucosa del abomaso. Esta pérdida de sangre también significa la pérdida de otros elementos figurados sanguíneos. Los animales con enfermedad parasitaria grave pueden presentar anemia, leucopenia por linfopenia y pérdida de proteínas que caracterizan una hipoproteinemia. Por lo tanto, métodos de evaluación del estado de salud de los animales fueron desarrollados como estrategia de ayuda en el control de Haemonchus. El grado de anemia de los animales del experimento fue determinado por el método Famacha e por métodos de laboratorio: albúmina, hematocrito, hemoglobina, proteína plasmática total, conteo total de eritrocitos, volumen globular, leucocitos totales, linfocitos y HPG (conteo de huevos en heces) en ovinos. El objetivo fue correlacionar el método Famacha con los resultados de los parámetros sanguíneos, para conocer de forma más rápido el estado de anemia del animal como indicador de hemoncose. Se ha utilizado para el experimento 20 ovejas mestizas de la raza Dorper. Las muestras de sangre, heces y la evaluación de la coloración de la conjuntiva ocular por el método Famacha se realizaron en el mismo día. Se ha observado un mayor número de animales con el grado de Famacha2, los animales con grado 1 presentaron menor número de HPG, mayor número de hematíes, mayor cantidad de hemoglobina, mayor hematocrito en comparación con los animales con grado 2 y 3. Se concluye que el método Famacha es eficaz en la estrategia de combate de la haemonchosis ovina.

ABSTRACT

The specific diagnosis and evaluation of the intensity of avian helminth infections are essential for efficacy studies and the determination of drug doses targeted to their control. This study evaluated the Mello and Campos method, originally described for parasitological diagnosis in dogs, in the recovery of scolices from cestode parasites of poultry (Gallus domesticus ). A total of 52 naturally infected birds obtained from farms underwent parasitological necropsy using the Mello and Campos method. The method consisted of four steps: content, soaking, scraping and evaluation. The number of scolices recovered per bird ranged from 1 to 4,345, and the highest number of scolices was recovered from material derived from the soaking step. The cestodes species diagnosed were Amoebotaenia cuneata , Choanotaenia infundibulum , Hymenolepis sp., Raillietina tetragona , Raillietina echinobothrida and Raillietina cesticillus . The Mello and Campos method, originally used to test for helminths in dogs, was effective in avian cestode testing because it includes a soaking step, which enables a more efficient recovery of scolices.(AU)


O diagnóstico específico e a avaliação da intensidade da infecção helmíntica em aves são fundamentais em estudos de eficácia e determinação de doses de medicamentos direcionados ao seu controle. O presente trabalho avaliou a aplicação e adaptação da metodologia de Mello e Campos, descrita originalmente para diagnóstico parasitológico em cães, na recuperação de escólices de cestódeos parasitos de aves domésticas (Gallus domesticus ). Foram empregadas 52 aves naturalmente infectadas e oriundas de produções rurais, as quais foram submetidas à necropsia parasitológica, adaptando-se a metodologia Mello e Campos. O método consistiu na realização de quatro etapas: conteúdo, imersão, raspado e avaliação. O número de escólices recuperadas por ave variou de 1 a 4.345, e o maior número de escólices foi recuperado do material oriundo da etapa de imersão. As espécies de cestódeos identificadas foram Amoebotaenia cuneata , Choanotaenia infundibulum , Hymenolepis sp., Raillietina tetragona , Raillietina echinobothrida e Raillietina cesticillus . Os resultados foram avaliados estatisticamente, concluindo-se que a metodologia adotada é eficaz para a recuperação de cestódeos de aves, uma vez que possui a etapa de imersão, que permite a recuperação mais eficiente de escólices.(AU)

ABSTRACT

Integrated control strategies are important for sustainable control of schistosomiasis and soil-transmitted helminthiasis, despite their challenges for their effective implementation. With the support of Good Neighbors International in collaboration with National Institute of Medical Research, Mwanza, Tanzania, integrated control applying mass drug administration (MDA), health education using PHAST, and improved safe water supply has been implemented on Kome Island over 5 years for controlling schistosomiasis and soil-transmitted helminths (STHs). Baseline surveys for schistosomiasis and STHs was conducted before implementation of any integrated control strategies, followed by 4 cross-sectional follow-up surveys on randomly selected samples of schoolchildren and adults in 10 primary schools and 8 villages, respectively, on Kome islands. Those follow-up surveys were conducted for impact evaluation after introduction of control strategies interventions in the study area. Five rounds of MDA have been implemented from 2009 along with PHAST and improved water supply with pumped wells as other control strategies for complementing MDA. A remarkable steady decline of schistosomiasis and STHs was observed from 2009 to 2012 with significant trends in their prevalence decline, and thereafter infection rate has remained at a low sustainable control. By the third follow-up survey in 2012, Schistosoma mansoni infection prevalence was reduced by 90.5% and hookworm by 93.3% among schoolchildren while in adults the corresponding reduction was 83.2% and 56.9%, respectively. Integrated control strategies have successfully reduced S. mansoni and STH infection status to a lower level. This study further suggests that monitoring and evaluation is a crucial component of any large-scale STH and schistosomiasis intervention.

INTRODUCTION: Spontaneous sedimentation is an important procedure for stool examination. A modification of this technique using conical tubes was performed and evaluated. METHODS: Fifty fecal samples were processed in sedimentation glass and in polypropylene conical tubes. Another 50 samples were used for quantitative evaluation of protozoan cysts. RESULTS: Although no significant differences occurred in the frequency of protozoa and helminths detected, significant differences in protozoan cyst counts did occur. CONCLUSIONS: The use of tube predicts a shorter path in the sedimentation of the sample, increases concentration of parasites for microscopy analysis, minimizes the risks of contamination, reduces the odor, and optimizes the workspace.


INTRODUÇÃO: A sedimentação espontânea é um procedimento importante para o exame parasitológico de fezes. Uma modificação desta técnica utilizando tubos cônicos foi realizada e avaliada. MÉTODOS: Cinquenta amostras de fezes foram processadas em cálices de sedimentação de vidro e em tubos cônicos de polipropileno. Outras 50 amostras foram usadas para avaliação quantitativa de cistos de protozoários. RESULTADOS: Embora não tenham sido encontradas diferenças significativas na frequência de helmintos e protozoários identificados, houve diferença significativa na contagem de cistos de protozoários. CONCLUSÕES: O uso do tubo prevê um caminho mais curto na sedimentação da amostra, aumenta a concentração de parasitas para a análise microscópica, minimiza os riscos de contaminação, reduz o odor e otimiza o espaço de trabalho.

Se realizó un estudio socioambiental y parasitológico en niños de áreas urbanas, periurbanas y rurales de las provincias de Buenos Aires y Misiones. Se completó una encuesta epidemiológica y se tomaron muestras fecales seriadas, las cuales se analizaron mediante técnicas de flotación y sedimentación. Del total de muestras fecales (1893), 5.5% fueron positivas en Buenos Aires y 25.6% en Misiones. El área periurbana de Misiones presentó la mayor prevalencia de parasitosis total (34.1%) y de biparasitados y poliparasitados (45.4%). Las especies más prevalentes fueron Strongyloides stercoralis (22.1%) en el sector periurbano y Ancylostomideos (21.6%) en el área rural de Misiones. Hymenolepis nana y Ascarislumbricoides mantuvieron bajas frecuencias en ambas provincias y Trichuris trichiura sólo se encontró en el periurbano de Buenos Aires. La prevalencia de geohelmintos aumentó con la edad (p < 0.05) y se asoció en el área periurbana de ambas provincias y rural de Misiones con las condiciones socioambientales: vivienda precaria, piso de tierra, eliminación de excretas en letrinas o a cielo abierto, sin recolección domiciliaria de residuos, madre sin actividad laboral, padres sin instrucción primaria (p < 0.05). Las características biogeográficas y las condiciones socioambientales de la provincia de Misiones favorecen la persistencia de las formas parasitarias en el ambiente.

ABSTRACT

Human intestinal capillariasis is caused by Capillaria philippinensis. This disease is endemic in Philippines and Thailand. To the best of our knowledge, we report the third case of human intestinal capillariasis from India and the first case from Andhra Pradesh, which is a non-endemic area. A 40-year-old female presented with diarrhoea, vomiting, decreased urinary output, ascitis, pedal oedema, hypoalbuminemia, and electrolyte imbalance. Microscopic examination of stool sample revealed the presence of ova, larvae, and adult worms of C. philippinensis. Patient recovered from the disease after taking albendazole 400 mg daily for 1 month along with supportive treatment.

This study compares the diagnostic accuracy of the TF-Test® (TFT) for human parasitosis with results obtained using the traditional Kato-Katz (KK), Hoffman-Pons-Janer (HPJ), Willis and Baermann-Moraes (BM) techniques. Overall, four stool samples were taken from each individual; three alternate-day TFT stool samples and another sample that was collected in a universal container. Stool samples were taken from 331 inhabitants of the community of Quilombola Santa Cruz. The gold standard (GS) for protozoa detection was defined as the combined results for TFT, HPJ and Willis coproscopic techniques; for helminth detection, GS was defined as the combined results for all five coproscopic techniques (TFT, KK, HPJ, Willis and BM). The positivity rate of each method was compared using the McNemar test. While the TFT exhibited similar positivity rates to the GS for Entamoeba histolytica/dispar (82.4 percent) and Giardia duodenalis (90 percent), HPJ and Willis techniques exhibited significantly lower positivity rates for these protozoa. All tests exhibited significantly lower positivity rates compared with GS for the diagnosis of helminths. The KK technique had the highest positivity rate for diagnosing Schistosoma mansoni (74.6 percent), while the TFT had the highest positivity rates for Ascaris lumbricoides (58.1 percent) and hookworm (75 percent); HPJ technique had the highest positivity rate for Strongyloides stercoralis (50 percent). Although a combination of tests is the most accurate method for the diagnosis of enteral parasites, the TFT reliably estimates the prevalence of protozoa and selected helminths, such as A. lumbricoides and hookworm. Further studies are needed to evaluate the detection accuracy of the TFT in samples with varying numbers of parasites.

ABSTRACT

The present study was performed to describe 2 human cases infected by the horsehair worm, Parachordodes sp., in Japan. Two gordiid worms were collected in the vomit and excreta of an 80-year-old woman in November 2009 in Kyoto city, and in the mouth of 1-year-old boy in December 2009 in Nara city, Japan, respectively. Both worms were males having bifurcated posterior ends and male gonads in cross sectional specimens. They were identified as Parachordodes sp. (Nematomorpha: Chordodidae) based on the characteristic morphologies of cross sections and areoles in the cuticle. DNA analysis on 18S rRNA partial sequence arrangements was also carried out and both worms were assumed to be close to the genus Paragordionus based on tree analysis, and far from Gordius sp. which has already been reported in humans in Japan. DNA sequencing of the Parachordodes worm does not appear on the database; therefore, more information on the gene sequences of the genus Parachordodes from humans, animals, or intermediates is required.

Las infecciones por microorganismos gastrointestinales constituyen hoy en día una de las principales causas de morbilidad y mortalidad en países en vías de desarrollo. Nos planteamos como objetivo evaluar la posible asociación entre la presencia de parásitos intestinales y la infección por Helicobacter pylori, y el comportamiento de anticuerpos séricos y secretores en una población infantil de la etnia Warao del Edo. Delta Amacuro, Venezuela. La presencia de parásitos se determinó por examen microscópico directo de las heces. Los niveles séricos de IgE total, IgG anti H. pylori e IgA anti Giardia duodenalis; y los secretores IgA total y específica a G. duodenalis y H. pylori en muestra de saliva, se determinaron utilizando el método de ELISA. El 65% de los niños estaban parasitados por protozoarios, observándose un 47% de poliparasitismo. Encontramos una mayor seroprevalencia de H. pylori en el grupo de niños no parasitados (46%) comparado con los parasitados (25%) (P<0,05). Sin embargo, los niños seropositivos a H. pylori y parasitados con G. duodenalis mostraron niveles séricos de IgE total mayores que los no parasitados (P<0,01); en contraparte, los niveles de IgA secretora total y especifica a H. pylori y G. duodenalis estaban disminuidos (P<0,05). Es posible que la respuesta inflamatoria generada por Giardia pueda aumentar los niveles de IgE total y disminuir la respuesta de IgA secretora favoreciendo la instauración de la infección por H. pylori.


The infections for gastrointestinal microorganisms represent nowadays one of the major reasons of morbidity and mortality in developing countries. We had evaluated both, the possible association between the presence of intestinal parasites and infection by Helicobacter pylori, and the production of serum and salivary antibodies in Amerindian Warao children from the Orinoco Delta, Venezuela. The prevalence of parasites was determined by faecal examination. The levels of serum antibodies (total IgE, specific anti- H. pylori IgG and anti G. duodenalis IgA) and salivary antibodies (total and specific IgA to G. duodenalis and H. pylori), was determined by ELISA. 65% of the child population was parasitized by protozoos, and a 47% of polyparasitism was observed. We found a major seroprevalence of H. pylori in the group of children not parasitized (46 %) compared with the parasitized ones (25 %) (P<0.05). Nevertheless, the seropositive children to H. pylori and parasitized with G. duodenalis showed levels of total IgE higher than the non parasitized ones (P<0.01); in contrast, levels of total and specific secretory IgA to H. pylori and G. duodenalis were decreased (P<0.05). It is possible that the inflammatory response generated by G. duodenalis infection may increase levels of total IgE and diminish secretory IgA response favoring the establishment of infection by H. pylori.

Introducción. La ascariasis hepática es la complicación menos frecuente de la infección parasitaria por el helminto intestinal Ascaris lumbricoides. Sin embargo, el cuadro de ascariasis biliar complicada con abscesos hepáticos se observa en ciertas zonas endémicas como América Latina y el Sudeste Asiático, donde debe ser un diagnóstico a considerar en pacientes con sintomatología biliar obstructiva, principalmente en niños, ya que por el tamaño pequeño de sus órganos tienen mayor probabilidad de desarrollar obstrucción. Este gusano tiene la habilidad de entrar en ductos, como la vía biliar intra y extrahepática, y al morir dentro de la misma provoca serias complicaciones como obstrucción, colangitis y abscesos secundarios. Caso clínico. Se presenta el caso de una niña de 6 años de edad con antecedente de Síndrome de Down, desnutrición proteico-calórica crónica grado II y nivel socioeconómico bajo, que se presentó con historia de fiebre diaria no cuantificada, tos seca, dolor abdominal y hepatomegalia dolorosa de dos semanas de evolución. Se realizó ultrasonido abdominal donde se encontraron múltiples abscesos hepáticos con A. lumbricoides en hígado y vía biliar. La paciente desarrolló un choque séptico sin respuesta al tratamiento instaurado y falleció a los 30 días intrahospitalarios. Conclusiones. La ascariasis hepatobiliar, aunque rara, es más común en países endémicos y tiene una alta morbi-mortalidad si no se diagnostica y se instaura el manejo adecuado tempranamente. Es imperativo que los médicos hondureños conozcan esta parasitosis para que con un adecuado manejo contribuyan a disminuir la mortalidad, principalmente infantil, por esta causa. Es necesario implementar medidas de prevención y control eficaces para las geohelmintiasis en Honduras...

INTRODUCTION: Children are an important high-risk group for helminth and protozoa infections. Daycare centers are environments where children have proven to be more susceptible to acquiring intestinal parasites. Thus, the purpose of this study was to verify the prevalence of intestinal parasites in children who attended the two daycare centers maintained by the local government of Uberlândia, State of Minas Gerais, Brazil. METHODS: Fecal samples were collected from 133 children (73 children at the Public Preschool for Early Childhood Education, PPECE A, and 60 at the PPECE B) following identification according to sex and age and agreement to participate by parents or guardians who signed the free, informed consent form. The samples were examined by the Lutz method. RESULTS: Coproparasitological tests performed on 133 children showed that 29.3 percent of them were parasitized for enteroparasites or commensals, 6.7 percent of the children presented polyparasitism. Among the protozoa, Giardia lamblia were the most prevalent and Hymenolepis nana were the most frequent among the helminths. CONCLUSIONS: Thus, analysis of the results showed that intestinal parasites still represent a public health problem, especially among children and in areas where the socioeconomic and educational conditions are less favorable.


INTRODUÇÃO: As crianças são importantes grupos de risco para infecções por helmintos e protozoários. Os centros de educação infantil são ambientes onde as crianças estão mais expostas à infecção por parasitas intestinais. O objetivo deste estudo foi verificar a prevalência de parasitas intestinais em crianças de duas creches mantidas pelo governo municipal de Uberlândia, Minas Gerais, Brasil. MÉTODOS: Os exames coproparasitológicos foram realizados em 133 crianças (73 crianças da Escola Municipal de Educação Infantil - EMEI A e 60 da EMEI B), depois da identificação da criança de acordo com a idade e sexo, e concordância dos responsáveis através do Termo de Consentimento Livre e Esclarecido. As amostras foram examinadas pelo método de Lutz. RESULTADOS: Os exames coproparasitológicos demonstraram que 29,3 por cento delas estavam parasitadas por enteroparasitos ou comensais e 6,7 por cento das crianças apresentaram poliparasitismo. Entre os protozoários, Giardia lamblia foi o mais prevalente, enquanto Hymenolepis nana foi mais frequente dentre os helmintos. CONCLUSÕES: Assim, podemos observar que as parasitoses intestinais ainda representam um problema de saúde pública, especialmente entre as crianças e em áreas onde as condições socioeconômicas e educacionais são menos favoráveis.

Plant root hairs are commonly found artifacts in parasitology specimens and may be confused with helminthes by an untrained eye. We report a case of brain tuberculoma where the tissue sample was contaminated with root hair derived from tap water; the presence of this root hair, which mimicked a larva, led to diagnostic confusion. Therefore, tap water should be considered a source of root hair and vegetable matter.
